Ethical Framework Essay

Ethics is a branch of philosophy that explores the ideal forms of human behavior, nursing a field rife with ethical dilemmas, application of nursing ethics can be objective or relative. It is obligatory to maintain a balance between the positive and negative aspects of treatments and options.

Medical indications: principles of beneficence and nonmaleficence

The condition of a patient is to be observed keenly to propose the treatment options. i.e. long-term diseases are treated with steroids. The primary purpose of any treatment should be to heal the underlying cause or alleviate the symptoms causing the agony. Although there are certain contraindications for treatment. According to an article published in the Journal of General Internal Medicine, which says “These decisions are particularly complex when physicians have less experience with these discussions, when families and providers disagree about benefits from treatment, and when cultural disparities are involved in misunderstandings” (Kasman, 2004). The success rate of a treatment or the efficacy of a medical play a rather eminent role, it is requisite to balance the pros and cons. The principle of beneficence plays a vital role in the well being of the patients, while the patient may be flummoxed but nurses and doctors are solely trained and educated for this very purpose.

The patient preferences: the principle of respect for autonomy

Autonomy is the freedom to make uncoated and informed decisions. Patient consent is an obligatory part of autonomy. According to the European charter of the patient’s right, “Every individual has the right to access all kinds of information regarding their state of health.” (Network, 2002) if a patient is refusing treatment, his wish Is to be respected unless the subject is shown to be mentally or legally incompetent. Legal incapacity is a rather specific matter. A patient has the right to lay out his values that may participate in the assessment of his mental state.

If the patient is competent his choice of treatment that could be suitable should be considered. If the patient is found to be incompetent, we then turn our heads towards a surrogate decision. According to a National study published in the Journal of the American Geriatrics Society, “Currently, up to 70% of community-dwelling older adults have completed an advance directive” (Teno, Gruneir, Schwartz, Nanda, & Wetle, 2007).

Nine Tenets essay

The nine provisions by Butts & Rich (2012) outline how nurses can be ethical in their practice. The two tenets from the 9 provisions that I plan to use in my practice include provision two and provision three. Provision 2 outlines that “The nurse’s primary commitment is to the patient, whether an individual, family, group, community, or population.” This tenet is important to me because it expresses the need to put the interests of the patient ahead of anything else. When conflicts of interests arise, they should be handled in a way that does not compromise patient safety and that the best interests of the patient are always preserved. An example of how this tenet is applied in practice is when nurses work overtime to receive bonuses not keeping in mind that overworking can lead to nursing burnout which can put the safety of patients at risk. In this scenario, nurses should not look at what they will gain financially by working overtime but should consider the implication it will have on patient safety.

Provision 3 outlines that “The nurse promotes, advocates for, and protects the rights, health, and safety of the patient.” This tenet is also important because it helps in promoting the rights of the patient. Patients have rights such as privacy and confidentiality which nurses should uphold at all times. Patients’ health records should not be disclosed to third parties without the consent of the patient. This tenet enhances my own nursing practice because patients are able to trust me with their information knowing it is safe with me. This improves patient satisfaction and leads to better patient outcomes. All the 9 provisions are important n nursing but these two provisions stated above stand out for me.

Advocacy Essay

Definition of the Dilemma

An example of a patient care situation where a nurse can intervene and advocate for a patient is when the patient’s right to autonomy and informed consent is being violated. A patient has a right to make decisions concerning his/her healthcare based on the ethical principle of autonomy. The nurse can advocate by ensuring the patient’s wishes are respected at all times and communicating these wishes to other parties involved in the care of the patient such as the physician or the patient’s family (Davoodvand, Abbaszadeh, & Ahmadi, 2016). The patient’s right can be at stake when it is not in line with the views of the physician or when some family members disagree with the choices made by the patient. In this scenario, the patient was to undergo a blood transfusion but was not willing to give his consent due to his religious beliefs. The nurse was left in a dilemma on whether to go ahead with the procedure or to respect the patient’s right to autonomy. The nurse had to act as the patient’s advocate by taking time to explain the benefits and risks involved with the procedure. The nurse, through the information provided, was able to convince the patient to undergo blood transfusion and this saved the patient’s life.

Medical Facts Relevant to the Dilemma

The medical facts that relate to this dilemma include patient autonomy and the duty of the nurse to ensure patient safety. Diagnosis and prognosis are also medical facts. Facts are important because they prevent the nurses from basing their decisions on assumptions only. The elements of informed consent are that the patient should have the mental capacity to make sound decisions, the nurse should disclose all necessary information regarding the procedure in question to the patient, the consent should be given voluntarily and the process should be well-documented. Medical care should not begin before a patient gives their informed consent. Some of the common assumptions that relate to this dilemma are

The non-medical facts that relate to this dilemma include cultural and religious influences. The patient was not willing to undergo a blood transfusion because he belonged to the Jehovah’s Witness denomination which was against the procedure. These facts often result in medical dilemmas because they conflict with some nursing principles.
