1. Must have Home Assessment Assignment Paper completed and each question must be answer in paragraph form of more than 3 sentences and very detail. (File Attached). Teacher want to be able to visualize the interviewed person as they are reading the report.

2. Overall report paper double space and type in roman numeral front. The overall paper must consist of everything  answered in step 1. 

3. The paper must have 3 resources and references( For each resources you must describe how each resource will benefit the client and what type of resource it is.)Example given was “Wheel on Meals”, and   ” Sneakers” at the YMCA  for older adults exercising. etc.

4.The paper must include a ethical dilemma( example given was patient want  take medication/ get medicine because of having to pay for rent.

Home Assessment Guidelines and Objectives

OBJECTIVES: The associate degree nursing student will demonstrate a holistic

understanding of a home assessment by:

1. Conducting an interview of an individual regarding physical, spiritual, cultural, social, and financial needs/issues.

2. Conducting a home assessment for potential safety risks.

3. Analyzing the assessment findings and providing evidence based solutions.

4. Discussing available community resources that can benefit the individual.

GUIDELINES:

1. Select an individual that is 55 years old or older who is disabled, or who is retired and 65 years old or older.

2. Describe the individual in detail. It must be descriptive of posture, appearance, articulation, mood and affect.

3. Describe individual’s neighborhood, the outside home environment, the inside of the home, and any actual or potential safety hazards.

4. Interview the individual regarding physical, spiritual, cultural, social, and financial needs/issues.

5. Discuss at least three community resources that benefit or could benefit the individual.

6. Discuss at least one ethical dilemma faced by the individual and solutions to the issue.

7. Provide evidence based resources to support your findings.

8. Use correct, grammar, and syntax.

Describe the individual in detail. Must be descriptive of posture, appearance, articulation, mood and affect.

The individual that I have chosen for this assignment is a 72 years old retiree. The old man stands at six feet tall but the advanced age has made him look shorter because of the bended back. He looks weak because of the problems and thoughts that keep haunting him. His body is malnourished and wrinkled making him appear even older than his actual age. His voice is freak and inaudible making it difficult to get the content of his speech. His mood is worried surrounded by deep thoughts, which makes his short tempered at the slightest of provocation hat one tries on him.

Describe individual’s neighborhood, the outside home environment, the inside of the home, and any actual or potential safety hazards.

The neighborhood is poor with majority of them peasant farmers unable to raise much for their families. There are many dumpsites located within the homes which many of the villagers go to scavenge for food. The safety hazard within the neighborhood is therefore the dumpsites that carry dangerous items and foul smell that keeps harassing the poor men and women from the region.

Interview and discuss the individual regarding physical, spiritual, cultural, social, and financial needs/issues.

The physical needs of the old man are a gym for his physical exercises. A church within the neighborhood will assist him to find solace in God after a day’s activities. There should also be an entertainment spot for the old man and others to meet and talk over a drink. Finances are mostly needed to supplement his diet.

Discuss at least three community resources that benefit or could benefit the individual.

A gym to help in exercising is crucial for the old man. Another resource is a guiding and counseling community center that the old man together with others will seek advice on issues that may be affecting them. Lastly, a senior assistance program may be helpful to the man since it will provide funds to purchase drugs and supplement his diet in order to live a healthy life.

Discuss at least one ethical dilemma faced by the individual and solutions to the issue.

The ethical dilemma that the old man faces is confidentiality. He is scared that the information may be leaked out to his disadvantage because he rarely shares his predicaments with others. The solution to this is to avoid asking intimate questions and promise to abide by the off the record remarks.
